There are descriptions of obesity and treatments for it for over one thousand years of human history, but as you can imagine, heretofore obesity was a disease of the ruling classes of society, the only strata where excess and luxury was possible really up until the industrial revolution. The first written record of medical therapy for obesity comes from Spain and the story of Sancho “the fat” of Leon. He took the throne of Leon upon the death of his elder brother in the year 956. Sancho had a famously ravenous appetite, eating up to seven meals a day, heavy in meat and bread. He reportedly weighed 21 arrobas, equivalent to about 530lbs.

Shortly after taking the throne, he was deposed by the local nobility because he was immobile, and more importantly, could not mount his horse. His grandmother took him to Moorish southern spain, to the court of the Caliph, where he was evaluated by court physician Hasdai Ibn Shaprut, a Jewish scholar and doctor.

Here was Sancho’s treatment-

- He was shut in a room for 22 hours a day, with his hands and feet tied to the bed posts.
- He was taken for a daily “walk” where he was dragged by ropes
- Subjected to long scalding steam baths
- His mouth was sewn shut such that it could only accommodate a straw
- He was fed seven times a day a concoction of blended up fruits and vegetables boiled in sea water, with various herbs and roots.
- As you can imagine, this led to endless diarrhea


After 40 days, he had lost half his body weight, weighing in at a svelt 240lbs. He could walk independently, ride his horse, consummate his marriage, and swing his sword. With the help of the Caliph’s army, he retook his throne in 960. He was reportedly a changed man after his experience under Dr. Shaprut’s care, eschewing his meat-heavy diet and opting instead for fruits and vegetables, until he was assassinated with a poisoned apple in 966.

Very effective, but perhaps a touch inhumane.

Had a friend who needed his jaw wired shut for a while after surgery. A liquid diet wasn't that bad, until he got sick.

Said that fortunately any fluids would still drain through the gaps in his teeth, and he only had to re-swallow the chunky stuff.

I knew a fat woman who had her jaws wired shut to lose weight. It did not work.
